We understand that for some individuals, adjusting to new stages in life can be challenging and may require additional support. At Rise2Shine Care, we offer a range of transition support services to assist NDIS participants in successfully navigating through these changes.

We assist people in making plans for change, including coming up with coping mechanisms for the stress that change may bring. Each participant receives a customised approach that takes into consideration their requirements and abilities. All of this is done to assist participants in achieving their short- and long-term objectives.

These transitions may include moving to a new home or shared NDIS accommodation, starting a new job, pursuing higher education, or returning to school.

  • In creating a budget plan to ensure timely payment of anticipated costs
  • In developing and executing strategies that ease the transition process
  • Establishing and sustaining peer support networks by mentoring participants and enhancing their decision-making abilities
  • Providing skills development programs that build capacity in a new environment
  • Providing financial guidance such as budgeting, planning, and saving.

As a registered NDIS provider, our support team at Rise2Shine Care are all ears for all that you have to say.
